Definition of Copyright | Works Protected by Copyright | Term

1. 1) Two (2) copies of a duly accomplished and notarized application form containing the following information:
   
   1. name and address of copyright owner;
   2. name of author and country of citizenship;
   3. title of the work (with edition number, if any);
   4. class to which the work belongs;
   5. date when the work or printing was completed, place of completion, name of person or establishment making the printing or reproduction;
   6. date when the work was placed on sale, sold or publicly distributed; place where sold or name of person or establishment who sold;
   7. name of address of person to whom certificate of registration shall be sent; and
   8. unit price, if reproduced in copies for sale.
2. Two (2) copies of the work as deposit, and the required registration fee and documentary stamps.
   
   1. If the work applied for registration is a published work, two (2) printed copies shall be submitted with application form. The following copyright notice should be printed in front or at the back of the title page if it is a book or on any conspicuous space if it is a non-book material:

      
      Philippine Copyright (Year of publication) by (Name of copyright owner)
      
      

   2. If the work applied for registration is an unpublished work, two (2) manuscripts and/or photocopies of the work without the copyright notice shall accompany the application.
   3. If the work applied for registration is an original ornamental design, a technical description of the design;
3. If the applicant is a non-resident foreigner, a Special Power of Attorney appointing a local authorized agent to prosecute copyright application for his behalf;
4. If a third party is claiming copyright ownership of the book, a duly notarized waiver of his copyright executed by the author in favor of the party making the claim;
   
5. If the applicant is a proprietor, a copy of certificate of business name should be attached to the application, and for a corporation, its SEC registration.
